Estradiol reduces breast cancer risk
*Epidemiological Studies:* bio-identical
1. *Lower risk with natural estradiol*: The Nurses’ Health Study (2013) found that women with higher natural estradiol levels had lower breast cancer risk (RR=0.74).
2. *Bioidentical estradiol*: The French E3N cohort study (2016) showed reduced breast cancer risk with bioidentical estradiol therapy (HR=0.68).
*Clinical Trials:*
1. *WHI Study* (2011): Estrogen-only therapy (conjugated equine estrogen) reduced breast cancer risk (HR=0.77) in postmenopausal women.
2. *KEEPS Study* (2013): Transdermal estradiol therapy (1-2 mg/day) showed no increased breast cancer risk.
*Mechanistic Studies:*
1. *Estradiol’s anti-proliferative effects*: Estradiol inhibits breast cancer cell growth and induces apoptosis (cell death).
2. *Estrogen receptor beta*: Estradiol’s affinity for ERβ, which has tumor-suppressive effects, may contribute to its protective effects.
*Timing and Dose:*
1. *Early initiation*: Estradiol therapy initiated early after menopause may reduce breast cancer risk.
2. *Low doses*: Transdermal estradiol (1-2 mg/day) may be more protective than higher
doses.
3. Pellets provide safest absorption and safety profile
*Contrasting Evidence:* use of non bio identical hormones like progestin contraindicated !
1. *Combined estrogen-progestin therapy*: The WHI Study (2002) found increased breast cancer risk with combined estrogen-progestin therapy.
2. *Synthetic estrogens*: Some studies suggest synthetic estrogens, like ethinyl estradiol, may increase breast cancer risk.
